Why was NONE not an option? I trust no one with this information!! They all sell it, share it, analyze it and then advertise based on it! N O !!! No one should keep it.

I read this as "from this list" - but agree that in the end, the trust just isn't there. One of the issues is, once they have it, they have it, there isn't reallly a 'do over'. And while things might be fine and dandy today - tomorrow there will be new threats, and new potential business reasons to share the data in some way.
Reminds me of that Benjamin Franklin quote - Three people can keep a secret if two of them are dead.

Should be reworded "trust most out of this list". The only one I really trust is "me".
Are we talking "desire" or "capability"? If the latter, I have zero faith in EHR Vendors (or a Health System, or a HIE)... Apple, hands down my "favorite".

l think you may get more than a few comments about the omission of 'None of the Above'. Trust is a big word when you're talking about sensitive information. I used to revel in my anonymity but that's little comfort now, when criminals go after entire databases.

Assuming that "protect" properly includes confidentiality, availability, and integrity, I wish you'd included "None of the Above" as a choice.
