    How about a recent ER visit in Georgia?

    1. Wrist band never checked (at reception, triage, or in the room).
    2. Left to carry stool sample from a bathroom back to my room through a hallway because the nurse was too busy to come and collect what she needed.
    3. Almost administered morphine despite listing it as an allergy in triage. I asked them(and promptly stopped them) just as they were cleaning my IV to administer the drug.
    4. No visible hand washing/sanitizing, even after handling stool and urine samples.
    5. EHR was left logged in when the nurse and doctor left the room. (I could see the patient list for everyone in the ED and easily could have clicked around.)

    On a plus, their waiting room chairs were very comfortable.

    They were recently acquired by a large healthcare org, so not sure if things will improve, but I was shocked at how bad things were. Writing a letter to their CEO and 'Safety VP' has given no response either...

    It's a toss up between my pharmacy and my insurance company. Pharmacy can't seem to figure out how to find my prescriptions in their system and they always have to look me up on 2 different screens - not sure what is going on there. Insurance company wouldn't let me provide them with my new home address and insisted that this information come from my employer. In the meantime they continued to send EOBs for at least a year to my old home address. But they somehow managed to send marketing material to my current home address.
