I pay a $149 annual "technology fee" to belong to One Medical. The visits are still covered by my Covered California (Obamacare) health insurance, but to go there at all, I have to pay the annual fee.

I do this for my 96 YO father. We could not find a decent PCP who accepted Medicare patients in a large population center. I went the concierge route for him and have a phenomenal PCP who visits his SNF and provides nearly 24/7 secure communication via Twistle.

I haven't done this for a PCP because all I go there for is a physical, but I would. I do however for many other services. I have a super high deductible, and many providers give me a better cash price than what I'd pay after processing through insurance.

I go out of network to get my preferred doctor. I am careful to have network doctor order costly labs and radiology tests, though. Some plans will deny them if not ordered by network provider. Another factor: doctors and insurers change contracts frequently. I don't care to switch providers each plan year.
