I've always worn a seat belt but in a big truck, it doesn't fit right or comfortably, so I slip the belt under my left arm. In Jun 2017, I got pulled over in Houston TX. $175 ticket!!! I wear it now with a flat clip that allows me to adjust it to fit properly.
