    By far, the largest mistake made this week by my local health system was to fax an insurance appeal to the wrong fax number, thus delaying a critical procedure from 1.5 weeks prior to hip surgery down to 3 days prior to hip surgery. Very frustrating. The only way it was uncovered was for me to make numerous phone calls to both the insurer and the provider. It was finally straightened out, but only after I was called to cancel the procedure because of missing pre-authorization. Is this why we did the whole electronic medical record thing (with incentives). so that we could continue to operate with 1980's technology? Experiences like this make me believe I wasted by entire 40-year career in healthcare IT.

    Accredo Pharmacy may be the most difficult organization I've ever seen and had to deal with. The time required to overcome their lack of information systems functionality is extreme and destructive to their client organizations, patients, and care providers.
