Based on my experience reviewing many medical malpractice matters, there are often many errors involved in addition to one or more patient-related issues. Without a thorough, independent investigation, it is very difficult to identify the main precipitating factors let alone assign blame. I think the results of these investigations should be made public so that others can learn from them.
