    I don't understand why Altman would be in any trouble. She wasn't involved in procuring drugs used by the dying veterans and, I believe, that American Law of Spousal Privilege, protects one from having to testify against their spouse. Furthermore, in a liberal state like Washington, I think that a good lawyer could persuade a jury to "choose not to apply the law", which juries have a right to do. Of course, this may be a scenario for next season; remains to be seen.
    Meanwhile, Owen and Teddy could go to Canada, where they could likely be licenced to practice. Since voluntary assisted death is legal in Canada, they could not be extradited since the extradition treaty only specifies that the reason for extradition has to be a crime in the country harbouring the extradition candidate, as well the country requesting them to be extradited.
    Since Canada has Universal Medical Insurance, it might be an opportunity to compare and contrast the two systems.
