What most limits your long-term career satisfaction in health system IT?

Not one of the options but lack of qualified leadership. I've been in healthcare IT for 22 years, and with each passing year I am more and more disappointed by the lack of good leaders -- people who engage thoughtfully with the industry and their surroundings, who are mission-driven, and who aren't just in it for a (very sizeable) paycheck. I can count on one hand the number of people I've worked with or for in a VP or above role who I thought earned their salary; everyone else is just cashing the check.

your ability to pick the right idiot. I wish there was a foolproof way to spot idiots but counterintuitively some idiots are very smart. They can dazzle you with words and misdirection, they can get promoted above you at work.
They can even be elected ;-)

I agree with the comment about leaving due to leadership. I have seen it time-and-time-again over my several decades in the industry.

I retired earlier than I intended (although in retrospect that's turned out well) because of a crappy department director. I had a long career in HIT and enjoyed almost all of the 18 years I spent at my last employer (a mid-sized acute care facility) until the last director I was under took over. Won't get into details but he was one of - if not THE - worst I ever worked under. I decided I'd had enough so I punched out.
