I chose regulators because they answer the question of who needs to take the first action to prevent this. Companies act based on incentives. They'll gladly swap out an executive for one who will be more cutthroat if it'll pay off with higher returns with only the threat of a slap on the wrist.
So morally it's on someone on the company side but pragmatically regulators are the missing piece that's letting this happen.

As long as industry election funding and regulatory influence remains as it is, there is little hope for real gain in cost, quality or access. Start with Big Pharma but it’s also Tech, Providers, etc. Check out open secrets.org.
