BT is all that´s really missing from the Nano.
But the development of watch-oriented accessories should stay with the 3rd party industry, that is even one of the cores of why and how the iPod is it´s own subculture.

I predicted the iWatch 4 years ago. And I just know 2011 is the year I finally get to see it.
I think there are really two questions here:
1) Can you wear the iPod nano well as a watch with third party bands or should Apple make a band
and
2) Is the iPod nano a good watch
For #1, I say third parties are doing a great job.
For #2, I think there's still a lot Apple can do to improve the "iWatch", including
• Making the screen stay on so you don't have to hit a button just to see the time
• More clock face options (maybe a plug-in system where you can design your own watch face)
• More time-piece features like World Time
• Bluetooth communications with iOS devices to:
– Display notifications like Caller Id, SMS, etc.
– Control the iOS music player
– Stereo Bluetooth
• And the holy grail: App Support to make your own time watch apps.

A few more hardware changes the iWatch needs:
• Small internal speaker for alarm
• Waterproof (at least water-resistant)
