I'd probably leave it to the patient with the organs to decide if he wanted to refuse the medicine so that the others could live or if he wanted to take the medicine.
I think this question is a bit moot as the AMA's ethical codes are pretty clear on the matter. I suppose the Hippocratic Oath can be left to some interpretation, though.
