No, I have not acquired/executed these documents.

Outdated, but 'Yes, I have all these docs. They are 10-years or more old.'

Outdated, but 'Yes, I have all these docs. They are 5-10 years old.'

Additionally, I also recommend my Patients update their necessary docs and remind them twice a year as a best practice.

Additionally, I never considered telling my Patients to update their necessary docs.
